General Requirements

The hours are 8:00 am to 4:30 pm, Monday through Friday, with hours on Wednesday until 8:00 pm for Passports only.

Due to economy and County budgetary constraints the vault will be closed Wednesday evening,starting August 19th 2010.

Then, print and submit the completed application form to the Passports area in the County Clerk's office.

Please check for any changes in office hours due to inclement weather or pre-holiday schedule.

  • All travelers must have their own passport.
  • Applicants must appear in person.
  • A parent or legal guardian must appear and fill out an application for a child under sixteen. The adult's ID will serve for the child.
  • An applicant must have a certified birth certificate with a raised seal from the Bureau of Vital Statistics or Department of Health and it also must state both parent's names.A Certificate of Naturalization or an expired or current passport will also be accepted.If one parent cannot appear in person he or she must submit a consent form along with a copy of the ID.
  • One color, 2" x 2" identical regulation passport photos taken within the last 6 months.
  • A valid driver's license for identification. If you do not have a valid driver's license, you must bring three forms of identification such as: a work ID, a student ID, other state issued licenses, a social security card, a current membership card, and/or a voter's registration card.
  • It will be necessary to bring a witness who can swear that they have known you for 2 years or more. The witness must present a valid driver's license . If in doubt, please call (973) 285-6161.
  • Social security numbers are mandatory for all persons over the age of two. Information on the parents is vital.
  • Children 15 years and younger must appear with both parents, properly identified, in order to execute a passport application. The child's birth certificates must be submitted and must be the one issued by the Bureau of Vital Statistics or the Department of Health, and it also must state both of the parent's names. Social Security numbers must be filled in on applications for children older than 2 years of age.
  • The fee for an adult passport is $135.00. Bring a check for $110.00 made payable to U.S. Department of State along with $25.00 in cash or check payable to Morris County Clerk.
  • The fee for a child's passport is $105.00. Bring a check for $80.00 made payable to U.S. Department of State along with $25.00 in cash or check payable to Morris County Clerk. Please note a child is considered to be anyone under the age of 16.
  • Separate checks are required for each passport.
